Visits can be scheduled by pay per visit model or by signing up for a monthly membership.
Individual medical visit without membership (Pay per visit):
- Single new patient consultation: $295 (60 minutes) / person visit
- Follow up & well child visits: $165 (30 minutes) / person visit
- TELEMEDICINE Consultation by Zoom: 60 minutes: $295 / 30 minutes: $165
Osteopathic Manipulative Therapy, Cranial osteopathy rates, 45 minute session:
- Member: $50 Adults / $25 Children
- Non-member: $200 / 60 minute session
Medical Marijuana Consultation
- Member: Included in Membership
- Non-member: $175
Direct Care Memberships (unlimited visits)
There is a $50 one time registration fee per family sign up. Minimum 4 month commitment requested. 30 day notice for cancellations. Membership is billed monthly with a card or bank account on file. Cash and checks accepted but discussed with Dr. Blahut prior to registration.
Individual Infant to 17 years of age (Monthly)
- $60 first child without a parent membership, $30 for each additional child
- $30 each child with a parent or family membership
Individual of 18 to 25 years old that is a full time student (Monthly)
- $70 without a parent or sibling member
- $50 with a parent/guardian member
Individual 26 to 99 years old (Monthly)
100 year olds and more are FREE to join with all the benefits
Group Family Rates (Monthly)
- 2 Parents and 1 child: $200
- 2 Parents and 2 children: $230
- Each additional child $30
- Living together couple (2 adults): $170
