DR. MICHAEL BLAHUT IS ACCEPTING TELEMEDICINE CONSULTATIONS FOR FUNCTIONAL MEDICINE. CURRENTLY ONLY TELEMEDICINE via ZOOM VISITS CAN BE BOOKED ONLINE, THESE ARE NOT IN-OFFICE VISITS.

Dr. Blahut can consult in you or your child's health from any state or country. He has a license to practice medicine in PA, CA, FL, & WY.

CONSULTATIONS AND APPOINTMENTS BOOKED HERE ARE NOT FREE, YOU WILL BE SENT AN E-BILL AFTER EACH VISIT. WE DO NOT ACCEPT INSURANCE, YOU MAY REQUEST A SUPERBILL TO SUBMIT TO YOUR INSURANCE FOR POTENTIAL COMPENSATION AFTER THE VISIT.

• Single new patient consultation: $295 (60 minutes) Telemedicine by Zoom
  
• Follow up visits: $165 (30 minutes), Telemedicine by Zoom
• Functional Medicine Membership: $249/mo, minimum 4 months.

We do not offer Primary Care services (well child visits).

We encourage all patients to use the online booking portal. Phone calls or messages usually require advanced notice. You may call 951-230-8315 and leave us a message for bookings or questions.

We request 24 hr notice for cancellations or a $50 fee may be applied to your account for no-show visits.
